Your primary care provider and their team includes one or two medical assistants, a full-time RN case manager who focuses on care coordination, and a case management support who provides case management support. Additionally, new members can be added or subtracted to your team as your health care needs change. Formed in to settle land claims, AFN continues to be the principal forum and voice of Alaska Natives in addressing critical issues of public policy and government. The U. Census Bureau is making good progress in rural Alaska, but there continues to be widespread confusion about how Native peoples should answer questions 5 and 9. As such, the Alaska Federation of Natives is undertaking a massive outreach effort in rural towns and hub-communities across the state. Each year, the AFN Convention draws some 6, attendees. The proceedings are broadcast live statewide, and webcast to 70 countries worldwide. Our Native people have a distinct trust relationship with the Federal Government and a unique relationship with the State of Alaska. We foster ongoing dialogue with our representatives and administration officials to advocate for fair and appropriate policies.
